Abstract
A 36 years old lady presents to the emergency department with left loin pain, nausea and vomiting. The pain has been increasing gradually over the past 24 hours with some fever and rigors. Her initial observations showed pyrexia of 38.6c, tachycardia 106/min and hypotension BP 95/30. On examination her abdomen was soft but tender in the suprapubic region. She was also tender in the left loin with tenderness over the left costovertebral angle.
